Community health centers in South Africa play a crucial role in providing accessible healthcare to underserved populations. Despite their importance, there is limited research on evaluating and improving these systems. Multilevel regression analysis was employed to examine clinical outcomes across multiple levels, including patient-specific factors and centre-level characteristics. Data were collected from a representative sample of community health centers over one year. Analysis revealed significant disparities in clinical outcomes between centers with varying resource availability and management practices. For instance, centers with higher staffing ratios had better patient recovery rates (65% vs. 42%, p < 0. 01). The multilevel regression analysis provided insights into the complex interplay of factors influencing clinical outcomes in community health centers. Future research should focus on implementing targeted interventions to address identified disparities and improve overall healthcare delivery. Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
