A peripherally inserted central catheter (PICC) is widely used for a variety of infusion therapies. Misposition, including primary and secondary, is one of the complications of PICC catheterisation. This case reports a neonate who encountered a PICC primary migration into the azygos vein (AV), which is very rare. In addition, we discuss related solutions after identifying this migration. AV migration for the patient can be detrimental, with an increased risk of perforation, thrombus and fistula formation. Therefore, we suggest that the case of malposition in the AV should be identified early and carefully.
