We report the case of a 53‐year‐old female with end‐stage osteoarthritis who underwent total hip arthroplasty via lateral decubitus direct anterior approach (DAA). Intraoperatively, a variant tensor fasciae latae (TFL) insertion was identified, characterized by an additional tendinous slip attaching directly to the greater trochanter, while the main TFL muscle belly continued distally toward the iliotibial band (ITB). The patient achieved a satisfactory functional recovery at 1‐year follow‐up. This case suggests that TFL‐ITB anatomical variations may not necessarily result in functional deficits and provides surgical insights for managing such variations during DAA procedures.
