Abstract Perigraft hygroma consists of sterile serous fluid accumulation around an intact vascular graft. We report a case of recurrent small bowel obstruction associated with a large perigraft hygroma following infrarenal abdominal aortic aneurysm repair with polytetrafluoroethylene (PTFE) graft. Definitive management involved open PTFE graft partial resection and interposition graft construction with Gelweave Dacron. This case highlights the rare development of small bowel obstruction due to perigraft hygroma and the role of graft replacement in the management of selected symptomatic patients.
