Las enfermedades neuromusculares (ENM) son un conjunto de patologías que afectan a la funcionalidad de los músculos esqueléticos y una de las principales causas de morbimortalidad en estos pacientes el fracaso respiratorio, secundario a disfunción muscular inspiratoria. Para el diagnóstico de la disfunción muscular inspiratoria se precisan la combinación de la capacidad vital (CV), la presión inspiratoria máxima en boca (PIM) y la presión inspiratoria nasal durante una inspiración máxima (SNIP). Valorar si es posible mediante tres técnicas de medición del tiempo de fonación máximo (MPT) definir la función pulmonar (CV < 50%ref) y la disfunción muscular inspiratoria (PIM<80cmH 2 O o SNIP<60cmH 2 O en mujeres y <70cmH 2 O en hombres) en pacientes con ENM. Se seleccionaron todos los pacientes con ENM que se les valoró la función pulmonar y muscular respiratoria entre junio 2020 hasta mayo 2024. Se midió CV, PIM y SNIP además de evaluar MPT mediante A-time, Crono Phonation Time (CPT) y Maximum Phonation Number (MPN). Se han incluido 92 pacientes con ENM lentamente progresivas (43 mujeres), edad de 61 ± 14 años; los diagnósticos fundamentales fueron miast enia gravis (28.7%), distrofia miotónica (22.8%), distrofia fascio-escapulo-humeral (19.6%). A-time, CPT y MPN correlacionaron positivamente y estadísticamente significativo con la CV. Solamente A-time correlaciono positivamente y estadísticamente significativo con la MIP. A-time y MPN correlacionó positivamente y estadísticamente significativo con la SNIP. A- time ≤13 s predicen una CV < 50%ref y disfunción muscular inspiratoria (sensibilidad 59%-especificidad 79%). CPT ≤15 s, predice una CV < 50%ref y disfunción muscular inspiratoria (sensibilidad 52%-especificidad 73%). MPN ≤ 18, predice una CV < 50%ref y disfunción muscular inspiratoria (sensibilidad 56%-especificidad 77%). A-time, CPT y MPN tienen valores que permiten definir el deterioro de la capacidad pulmonar y de la función muscular inspiratoria que indican la necesidad de ventilación mecánica domiciliaria en los pacientes con ENM lentamente progresiva.
