This study presents the Xu imbrication flap, a novel method for earlobe cleft repair. The earlobe cleft, especially the congenital type, affects aesthetics and may cause psychological issues. Existing repair methods have drawbacks, such as scars and asymmetry. The Xu imbrication flap involves creating rectangular flaps on both sides of the cleft and may include Z-plasty adjustment. In a 12-year-old girl with a congenital earlobe cleft, this method achieved good results. Postoperatively, the earlobe had a smooth curve, with no obvious scars or notches. The advantages include preserving tissue volume, reducing scars, preventing notches, simplicity, and regulating tissue volume distribution. Therefore, the Xu imbrication flap is an innovative approach worthy of clinical promotion.
