Dear Editor, The renal arteries typically branch off the abdominal aorta at a right angle, just beneath the origin of the superior mesenteric artery (SMA), which is located between the intervertebral discs of the L1 and L2 vertebrae. However, it is quite frequent to observe variations in both the origin and the course of the renal arteries. With the rising frequency of kidney transplants and other urological procedures, surgeons must have a heightened awareness of the variations in the surrounding vascular anatomy. It is essential to examine the renal arteries using computed tomography angiography (CTA) before any surgical procedure, such as kidney transplants, conservative or radical kidney surgeries, or treatment for renal trauma. This step helps prevent the potential complications or avoidable health risks. The abnormal positioning of renal arteries is a result of the varying positions of the kidneys during their development. Once an organ or structure assumes its typical position in the abdomen, its blood supply also undergoes a sequential change. The variations in the origin of renal arteries can be attributed to the development of the mesonephric arteries. We describe the two cases of anomalous renal artery origins. In the first case, the right renal artery originates from the aorta, situated above the point where SMA branches off Figure 1 In the second case, the left renal artery arises from the aorta, positioned below the origin of the inferior mesenteric artery Figure 2. In cases of anomalous origins, prior knowledge acts as a guide for planning the procedure. In conclusion, increasing awareness of anomalous renal artery origins through CTA significantly contributes to better surgical planning and minimizes the likelihood of complications.
