Abstract Background and aims Stroke is one of the leading causes of death and disability worldwide, posing a major economic and social burden on patients and society. In Andalusia, Spain, it remains the primary cause of mortality. Its occurrence is strongly influenced by both non-modifiable and modifiable risk factors, many of which coexist with chronic diseases. Exploring these relationships is crucial for guiding prevention strategies and optimizing resource allocation. The aim of this study is to assess the association between chronic conditions and stroke incidence. Methods The dataset includes diagnoses of chronic diseases from 406978 patients of Andalusia, half of the patients has suffered a stroke between 1999 and 2024 while the other half were selected as controls, matched by age distribution and sex. For identifying the chronic diseases diagnoses with significant association with stroke a chi-squared hypothesis test was performed. Results The chronic diseases that presented significant association with stroke were poorly defined and other cerebrovascular diseases, sequelae of cerebrovascular disease, occlusion or stenosis of precerebral arteries, neurological disease with motor deficit not due to stroke, atrial fibrillation, congenital cardiac and circulatory anomalies. While other chronic diseases classically identified as risk factors for stroke, such as high blood pressure, dyslipidemia, diabetes, ischemic heart disease, obesity or smoking did not show significant direct association. Conclusions There is likely to be greater control, redefinition, or quantification of classic cardiovascular risk factors over time. Other factors, such as acute-phase stroke precipitants or those related to the exposome, will need to be defined.
