One of the most important medical and social issues is that of increasing number of elderlies with polymorbid conditions. It leads to complications in socio-economic relations, deterioration of the population’s health, and a decline in quality of life. Inter-agency collaboration is considered as the foundation for developing effective strategies aimed at improving the processes of identifying and supporting people in need of care. The purpose of the study was to assess the effectiveness of a long-term care project for the elderly and disabled being implemented. Materials and methods. A cross-sectional sociological study was conducted. There was developed a questionnaire including several blocks of questions addressed social workers in Saint Petersburg. There were 334 participants who represented 18 districts of St. Petersburg. The findings demonstrated ineffective implementation of the project, in particular, more than a quarter of respondents indicated that they were not involved in the implementation of this project. Only 14 people (4.19%) noted that the mechanism for identifying elderly in need of care is well developed. Conclusions. There is a need for the immediate rectification of identified shortcomings and the enhancement of interdisciplinary and inter-agency approaches in the management of older patients.
