Esophageal cancer metastasizing to the prostate is extremely rare, with only sporadic cases reported in the literature. Here, we present a case of esophageal cancer with prostate metastasis detected by 18 FFDG PET/CT in an 84-year-old man with multisite metastatic disease. 18 FFDG PET/CT demonstrated markedly increased glucose metabolism in both the primary esophageal lesion and the prostate, enabling the detection of this uncommon metastatic site. This case underscores the important role of 18 FFDG PET/CT in accurately identifying atypical metastatic patterns, thereby enhancing diagnostic precision and informing clinical staging and management strategies in patients with advanced malignancies.
