BACKGROUND: This paper aims to review and critically analyze current nuclear medicine procedures able to assess the treatment response to 177LuLu-PSMA, highlighting their strengths and limitations, as well as future developments.METHODS: A comprehensive literature search was made by using PubMed and Scopus databases, until August 2025. The following search strings were employed: A: “prostate cancer” OR “prostate neoplas*” OR “prostate malignan*”; B: “Lutetium” OR “177Lu-PSMA” OR “PSMA therapy” OR “Lutetium-177 PSMA” OR “LuPSMA” OR “177Lu-PSMA-617” OR “177Lu-PSMA-I however, a highly heterogeneity across the studies required prospective trials.
