Abstract Rationale Obstructive sleep apnea (OSA) is a prevalent disorder associated with cardiometabolic and neurocognitive morbidity. Despite widespread use of continuous positive airway pressure (CPAP) therapy and the growing availability of connected health devices, large-scale real-world data describing daily sleep, activity, and physiological patterns in adults with OSA remain limited. Establishing such benchmarks is essential to contextualize population health trends, evaluate treatment adherence, and inform digital health interventions. Methods Wearable device data were retrospectively analyzed for 312,547 adults using CPAP therapy for OSA management. Participants were aged 18 years or older and met quality thresholds of at least 30 valid sleep nights (3-14 hours per night) and at least 45 valid physical activity days (≥100 steps per day) within a 365-day window ending on October 15, 2025. Aggregated variables included total sleep duration (minutes, min), sleep efficiency (percentage of time asleep while in bed), exercise (min per day), daily step count, body mass index (BMI), resting heart rate (beats per minute, bpm), heart rate variability (HRV, milliseconds, ms), and blood oxygen saturation (SpO2, %). Results were summarized overall and stratified by sex, region (North America, Australia/New Zealand (ANZ), East/South/Southeast Asia, and Latin America), and season. Values are presented as mean ± standard deviation. Results The cohort (Table) comprised 67% male, 32% female, and 1% other/unknown participants, who identified as another sex or did not report their sex. with a mean age of 52.6 years. Average total sleep time was 398 ± 63 min, with a mean sleep efficiency of 89.9 ± 11.6%. Participants averaged 5,837 ± 2,701 steps/day, with the highest activity levels in Asia (6,992 steps/day) and the lowest in North America (5,786 steps/day), paralleling lower BMI in Asia (28.7 kg/m²) versus North America (32.9 kg/m²). Females demonstrated slightly higher sleep time (405 min vs. 394 min) and HRV (37 ms vs. 32 ms) compared with males. Results were consistent in a sensitivity analysis of those with ≥5 valid sleep nights per season. Conclusions This analysis provides the first global, real-world benchmarks of sleep, physical activity, and physiological parameters in adults with OSA. Regional variability in sleep, activity, and physiology point to lifestyle and environmental influences, highlighting opportunities for further investigation and personalized OSA management. These findings lay the groundwork for leveraging connected-health ecosystems to explore behavioral and physiologic variability, develop digital biomarkers, and optimize treatment pathways.
