ABSTRACT Background: Decreased brain and spinal cord perfusion that leads to infarction is known as a stroke, and it is one of the most common causes of death worldwide. Among young children and adults, stroke can be a sickle cell disease (SCD) complication. Management and prognosis of stroke depend on the type and location of the stroke. However, there is a lack of research on the distribution of stroke among SCD patients. We aim to identify the stroke location among SCD patients in King Abdulaziz University Hospital (KAUH), Jeddah. Methodology: A retrospective cohort study was conducted at the KAUH in July 2023 using extracted data from SCD and stroke patients between 2010 and 2023. Data entry was performed using Microsoft Excel 2016, while statistical analysis was performed using the Statistical Package of the Social Sciences (SPSS) version 21. Results: A total of 86 patients were included in the study, with a mean age of 42 ± 10.64 years. Among patients with ischemic stroke, 59.3% ( n = 51) had occlusion in the anterior circulation, while 18.6% ( n = 16) involved the posterior circulation. In cases of hemorrhagic stroke, 18.6% ( n = 16) were due to rupture in the anterior circulation, and 3.5% ( n = 3) were related to the posterior circulation. Hypertension was identified as the most common associated risk factor. Conclusion: Patients with SCD and hypertension are more susceptible to developing stroke within the anterior circulation.
