Colorectal cancer liver metastasis (CRLM) with concomitant hepatocellular carcinoma (HCC) in a single patient has not been previously reported. We present a case of a 57-year-old Asian male with a rectal mass and synchronous, radiologically distinct multiple liver lesions. Contrast-enhanced ultrasound (CEUS) characterized the liver lesions as suggestive of both CRLM and concomitant HCC, which was later histologically confirmed by ultrasound-guided biopsy. This case underscores the significant diagnostic value of CEUS in differentiating complex multifocal liver lesions and the critical role of targeted biopsy for definitive diagnosis.
