Introduction and Objective: YAs experience many life changes that impact diabetes self-care. In an era of increasing AID use, it is timely to study the interplay of AID use, sociodemographic factors, and glycemic control in this vulnerable age group. Methods: Electronic health record data from 2022-23 were reviewed for YAs with T1D at a tertiary center. YAs ages 18-24 at 1st visit were followed until age 26 or the end of 2023. Each visit was assessed qualitatively for consistency of AID use and reported reasons for AID use/non-use. We created User Profiles based on dominant AID use patterns: 1) Consistent Users, 2) Inconsistent Users, 3) Discontinuers, and 4) Never Users. Sociodemographic characteristics, glycemic outcomes from YAs' most recent visit, and reported reasons for AID use/non-use were compared among User Profiles. Results: Data included 1142 YAs (age 22.6±2.1y, T1D duration 13.0±5.8y, 50% male, 92% on private insurance, 91% CGM users, 73% pump users, A1c 7.6±1.4%). Notable differences emerged in characteristics and glycemic outcomes based on AID user profiles (Table). AID non-users most often endorsed personal reluctance to use AID; Inconsistent Users most often reported supply issues and challenges staying in automode. Conclusion: Differences among AID user profiles indicate suboptimal glycemia and health parity challenges in those not using AID.
