This paper presents PromptLedger, a pre-execution AI governance platform implementing a nine-layer deterministic governance architecture for regulated enterprises. The nine layers are: Intent Capture, Pre-Execution Governance, Policy and Authority Check, Risk Classification, Execution Boundary and Reasoning Oversight System (ROS), Model Call, Output Validation, Immutable Audit Log, and Human Escalation Gate. The architecture enforces policy, classifies risk, validates output, and maintains an immutable append-only audit log with SHA-256 hash chain verification before, during, and after every AI model call. Key innovations include acceleration-based escalation triggers using derivative behavioral monitoring, evidentiary graph architecture for AI output auditability, Interruption Realism Verification, and a context-aware semantic Output Gate. The platform is live at promptledger.co. Six provisional USPTO patents have been filed covering the core architectural innovations (Application Numbers 64/057,163, 64/057,231, 64/057,233, 64/063,384, 64/074,178, 64/074,180).
