Background Adenomatoid odontogenic tumor (AOT) is a benign odontogenic tumor typically arising intraosseously in adolescents and often associated with impacted teeth. Case presentation A 14-year-old girl presented with painless swelling in the anterior maxillary gingiva. Radiographs showed a well-defined radiolucency between the maxillary right central incisor and canine with localized cortical bone loss. The lesion was excised under general anesthesia, and histopathology confirmed AOT. Conclusion This atypical presentation expands the clinical spectrum of AOT and underscores the importance of careful radiographic evaluation of gingival swellings in adolescents. In patients with developmental disorders, individualized behavioral assessment and treatment planning may be required to ensure safe diagnosis and management.
