This study assessed the implementation of the ISO 9001:2015 Quality Management System (QMS) at President Ramon Magsaysay State University (PRMSU). Using a descriptive-quantitative design, the study gathered responses from 210 university personnel across seven campuses and 130 delivery units, including delivery unit heads, staff, document control officers, internal quality auditors, and other personnel. A validated survey questionnaire measured implementation in six dimensions: leadership and commitment, planning and risk management, support and resource allocation, operation and service delivery, performance evaluation and monitoring, and continuous improvement. Results showed that all dimensions were fully implemented, with a grand mean of 3.71. Leadership and commitment obtained the highest rating, while support and resource allocation ranked lowest but remained fully implemented. ANOVA results indicated no significant differences when grouped by sex, age, civil status, and length of service; however, significant variations emerged by position and campus assignment, showing uneven implementation experiences across organizational roles and geographic units. The study concludes that PRMSU has established a strong QMS foundation, but multi-campus harmonization, resource access, document control, and real-time monitoring require further strengthening. A strategic intervention program is recommended to institutionalize cross-campus mentorship, risk-management capacity building, a cloud-based document management system, and a real-time internal quality audit dashboard.
