CANONICAL STATUS This publication is a CORE-equivalent foundational extension of MAV-MCC 4S+4T. It does not overwrite the prior canonical v0.4.0-R2 record; it freezes a new derivation with independent provenance and equivalent foundational weight. CONTRIBUTION The record formulates a corrected minimal effective action in which the 4S+4T register is represented as an eight-dimensional internal state geometry over ordinary 3+1 spacetime. A complex order parameter describes a coherent Primary Phase Network. A transition of this state can, in principle, release configuration energy through gauge-invariant Standard-Model channels, thermalise into a hot bath, and enter a quark-gluon-plasma regime above the QCD crossover. The same transition provides a single energy ledger for visible excitations, a stable hidden-matter candidate, residual or slowly relaxing dark-energy-like configuration energy, gradients, and gravitational radiation. The Hot Big Bang is therefore treated as the observable energetic and thermodynamic regime generated by a global state transition, rather than as an assumed absolute creation event. The proposed frequency-to-matter cascade is: coherent primary phase → quantised field excitations → quarks and gluons → quark-gluon plasma → hadrons → nuclei → atoms and complex matter. QUANTUM CORRELATIONS AND OBSERVATIONAL PROJECTION Entanglement is defined through the initial density operator and a causal influence functional rather than as an additional local force. Observable 3S+1T data are modelled as a non-invertible projection of 4S+4T dynamics, allowing part of the generative information to remain directly unobservable while leaving structured residual correlations, memory, species dependence, and projection deficits. BIOLOGICAL-LIMIT COROLLARY A restricted biological-limit corollary describes living systems as driven open systems capable of maintaining, selecting, disrupting, and restoring functional coherence. It supplies equal-energy phase-scrambling, phase-rescue, finite-dephasing, retention, feedback, and repair predictions without claiming an established general quantum theory of life. DOUBLE AUDIT The formal audit gives a conditional PASS as a local low-energy effective field theory after correcting the interpretation of 4S+4T, restoring electroweak gauge invariance, separating entanglement from the local action, enforcing positivity and boundedness conditions, and preventing dark-energy double counting. The empirical audit gives a PASS for publication as a falsifiable proposed framework, but a FAIL as an established explanation until a constrained parameter point jointly reproduces cosmology, quark-gluon-plasma thermodynamics, dark-sector behaviour, and the frozen out-of-sample predictions. PREDICTION FREEZE An exploratory ALICE identified-hadron flow-fluctuation calibration is included but permanently excluded from confirmatory scoring. It motivates a fixed independent QGP endpoint: in 1 < pT < 3 GeV/c, the baryon-minus-meson relative-v2-fluctuation branch must have a centrality slope less than or equal to -0.015 per ten centrality percentage points, while the latent model must improve both complexity-penalised and held-out prediction. Additional frozen directional predictions cover the phi/proton discriminator, longitudinal retention, harmonic hierarchy, common energy accounting, suppression of independent phase domains, dark-sector behaviour, causal quantum correlations, and biological phase controls.
