Computer-aided diagnostic (CAD) systems are a promising tool to increase this accuracy. The aim of this study is to assess the diagnostic performance of a CAD system based on 3D multiparametric transrectal prostate ultrasound (3D mpUS) for the detection of clinically significant prostate cancer (csPCa). METHODS: The evaluated CAD system was developed using artificial intelligence and trained using 3D mpUS images from "positive" patients undergoing radical prostatectomy (RP) and "negative" patients with suspicion for PCa but with negative MRI (PIRADS≤2) or negative systematic biopsies. 3D mpUS consisted of 3D grayscale, 4D contrast-enhanced ultrasound, and 3D shear wave elastography. Histopathology of RP specimen was used as ground truth, csPCa was defined as Gleason group≥2. Performance was assessed by comparing the CAD prediction with the ground truth on the US images. Visual assessment determined if targeted biopsies would result in detection of csPCa. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were calculated on lesion level (any lesion of csPCa) and on index lesion level (lesion with highest ISUP of≥0.5 cc). RESULTS: Based on a retrospective validation of 220 patients, the sensitivity and specificity were 81.2% (74.1 – 87.0) and 39.3% (27.1 – 52.7) on lesion level for the detection of csPCa, with a ROC AUC of 0.60 (0.53 – 0.67). The PPV and NPV were 77.3% (69.9 – 83.4) and 45.1% (31.6 – 59.6). On index lesion level, the sensitivity was 74.7% (67.1 – 81.3). For csPCa defined as Gleason Group≥3, the sensitivity was 88.4% (80.5 – 93.8). This study is limited as the study cohort holds a significantly higher prevalence of csPCa than a general population. Moreover, the performance was assessed through visually simulating the target biopsy procedure and not by taking actual targeted biopsies. CONCLUSIONS: The diagnostic accuracy of a CAD system based on mpUS shows robust (simulated) detection of PCa at (index) lesion level and is similar to that of MRI. Prospective clinical validation is needed to further investigate the non-inferiority compared to MRI.
