Purpose This study aims to identify the prospects, challenges and future directions in blockchain technology-based healthcare system. Design/methodology/approach A systematic literature review (SLR) was conducted to address the study’s objectives. A total of 3,987 studies were explored through 12 electronic databases, including Web of Science and Scopus. Of which, 25 most relevant seminal studies published in peer-reviewed journals from 2015 to 2024 were selected to conduct the study. Findings Findings of the study revealed that major prospects of blockchain in healthcare included health records management, data integrity, data sharing and collaboration, and efficiency and transparency. The results revealed that integration challenges, technical limitations and security issues caused barriers for the incorporation of blockchain-based healthcare systems. The results also manifested that standardization and interoperability, privacy and security measures, technological innovation and collaboration provided a comprehensive roadmap for advancing the adoption of blockchain technology in the healthcare sector. Originality/value This study has added valuable literature to the existing body of knowledge. It has also provided future directions to efficiently adopt blockchain technology in healthcare sector.
