METHODS: We retrospectively evaluated 1406 patients with MIUC who were treated with radical cystectomy (n=771) or radical nephroureterectomy (n=620). We selected patients with pT2 or ypT2 and compared disease-free survival (DFS) and overall survival (OS) between the groups. The effect of LVI status on outcomes were evaluated by multivariable Cox regression analysis. RESULTS: Of 1406, we identified 169 and 154 patients with pT2 and ypT2, respectively. DFS and OS were significantly shorter in the ypT2 group than those in the pT2 group. A poor DFS and OS was observed in the patients with pT2/ypT2 and LVI+compared with LVI-. We observed similar median DFS (95.3 vs. 96.6 mo.) and OS (96.7 vs. 96.4 mo.) between the LVI+in pT2 and LVI- in ypT2. Multivariable Cox regression analysis showed LVI+was significantly associated with poor DFS and OS. Nomogram for 5-year DFS showed LVI+was associated with 14% reduction of survival probability. CONCLUSIONS: The impact of LVI on oncological outcomes was significant in both pT2 and ypT2 MIUC.
