Background and Issues: Hyperacute telestroke care requires synchronization of multiple teams and systems. Gaps in processes, such as logistics, workflow, and technologies can contribute to delays in patient care. Delays in stroke care can decrease the likelihood of excellent clinical outcomes. To track, monitor, and reduce process gaps, our telestroke practice created a quality flagging system within our internal electronic medical record. Providers can flag issues such as, “camera not with patient” or “unable to access imaging,” in real time. The clinical operations team then reviews a list of quality flags. As the volume of telestroke consults increased, the team identified a need to track quality flags in a scalable and sustainable manner. Purpose: Our aim is to establish a dashboard that enables efficient analyses of the most frequent quality issues flagged by our providers, without the need for manual review or calculations, to be used for data-driven, systematic process improvement. Methods: A Power BI dashboard was created to allow both broad and focused analysis. Quality flags can be explored at either the health system or facility level, and can be filtered by patient location or consult type for any specified time period. The dashboard includes two components for analysis: a Pareto chart which aggregates the most prevalent quality flags in decreasing order of frequency, and a series of graphs showing trends over time for each categorical quality flag. Results: The dashboard is a valuable tool for aggregating and analyzing quality flag data. The clinical operations team can determine which issues need prioritization for telestroke process improvement. Prior to dashboard creation, the team used quality flag data retroactively on a case-by-case basis; now the dashboard is used proactively to drive process improvement for 75% of clients. Conclusions: Provider-submitted quality flags are a key source of information on process issues that occur during telestroke consultations. A Power BI dashboard is an ideal tool to manage and translate quality flags into actionable data. The clinical operations team can target process improvements toward the most prevalent issues, and track effectiveness of interventions over time.
