In order to gain a deeper understanding of the mental health status and influencing factors of adolescents visiting child and adolescent psychological clinics in Shanxi Province, this study conducted a questionnaire survey on mental health symptoms such as anxiety and depression among adolescent patients aged 10 to 19 who visited the psychological clinics of Shanxi Children’s Hospital (Shanxi Maternal and Child Health Hospital) from June 2020 to June 2024. Data quality control was implemented across three key stages: questionnaire distribution, survey implementation, and data collection and organization. On this basis, a total of 459 valid questionnaires were retrieved. The survey results revealed that 91.3% of the adolescents in the outpatient department reported mild to severe anxiety symptoms, and 92.8% reported mild to severe depressive symptoms. This dataset has wide applicability and supports in-depth exploration and data mining using a wide range of statistical analysis software and analytical techniques. The empirical evidence derived from this dataset can provide valuable support for researchers and policymakers. It can not only promote further research in related fields but also help improve scientific policy measures to safeguard adolescent mental health, and contribute to social harmony and stability as well as sustainable national development.
