Rural communities in Ethiopia face significant barriers to accessing mental health services, which can exacerbate existing social and economic challenges. Community focus groups were conducted with a total of 50 participants from four randomly selected rural villages. Focus group discussions covered topics such as perceived barriers to accessing mental health care, preferred types of services, and current service delivery mechanisms. Focus group analysis revealed that the primary barrier to accessing mental health services was geographical distance (mean distance: 20 km), with a proportion of 75% of participants citing this issue. Additionally, stigma associated with mental illness was prevalent among community members, influencing their willingness to seek help. The findings highlight the critical need for improved transportation infrastructure and culturally sensitive intervention programmes to enhance service accessibility in rural Ethiopian villages. Rural health facilities should be strategically located closer to communities, and targeted awareness campaigns should aim to reduce stigma surrounding mental health issues. Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
