Multilevel lumbar stabilization surgery can cause considerable postoperative pain. The quadro-iliac plane block (QIPB) is a novel interfascial technique that may support multimodal analgesia. In this case series, six patients received bilateral ultrasound-guided QIPB after the induction of general anesthesia. Resting and movement Numeric Rating Scale scores remained ≤3 and ≤4, respectively; 24-hour morphine use was 4 to 8 mg. Quadriceps strength and QoR-15 scores were well-preserved, and no block-related complications occurred. The QIPB may act as a potentially motor-sparing adjunct in multimodal postoperative analgesia; however, controlled trials are needed to confirm this.
