ABSTRACT In young adults who experience sudden flank or abdominal pain and hemodynamic instability following intense physical activity, spontaneous retroperitoneal hemorrhage due to an iliac arterial lesion must be considered, even in the absence of evident contrast extravasation on CT angiography, requiring immediate endovascular assessment and damage‐control resuscitation.
