AbstractSIP-AI-03 established excretory architecture for AI memory systems. But agentic AI does notmerely know; it acts. Memory coherence is metabolic; action coherence is executive. An agent thatacts on a stale world model, pursues contradictory goals, or executes irreversible actions withoutgovernance is dangerous.This paper extends the excretory architecture to persistent action-taking AI. We introduce: (1)continuous world model maintenance with confidence decay and reality anchoring; (2) the ActionJournal with reversibility classification and cryptographic immutability; (3) goal stack hygienethrough contradiction detection and prefrontal override; (4) the Actor/Steward architecture for multi-agent coherence; and (5) the Extended Librarian Function governing action approval.The reversibility taxonomy (R0-R4) is context-sensitive: consumer assistants operate with liberalautonomy; defence systems require multi-party authorisation. When goals contradict, the agentfreezes and requests direction.
