Abstract Lichen planus pigmentosus (LPP) is a disorder of pigmentation usually involving the face, neck, and upper trunk. Rare presentations, including zosteriform, mucosal, and linear, have been reported in literature. Herein, we report a case of young female who presented with hyperpigmentation around both eyes for 6 months. Examination revealed homogenous diffuse slate gray pigmentation involving periocular region. Dermatoscopic evaluation of the lesion under polarized light demonstrated brown-black pigment dots and globules with sparing of the eccrine openings. On the basis of clinical examination and dermatoscopic evaluation, a diagnosis of periocular LPP was made. This case highlights the importance of dermoscopy in diagnosing pigmentary disorders over cosmetically important sites.
