The quadro-iliac plane block (QIPB) is a recently described interfascial plane block used for postoperative analgesia after lumbar spine surgery. We report a case of a 52-year-old morbidly obese woman (body mass index BMI 42.3 kg/m 2 ) who received bilateral ultrasound-guided QIPB using 30 mL of 0.25% bupivacaine per side. The iliac crest provided a consistent sonographic landmark despite increased soft tissue depth. Numeric rating scale scores at rest and during activity remained ≤2 throughout the postoperative assessments, with a total 24-hour intravenous morphine consumption of 2 mg. No motor block and complications were observed. Serial sensory assessments demonstrated region-specific, time-dependent dermatomal blockade.
