Complicated crown fractures are emergency situations in dentistry that are frequently encountered in anterior teeth after trauma, especially in childhood, and result in pulp exposure. Pulp-preserving treatments are crucial for the long-term prognosis of the tooth. This case report describes the treatment process of an 8-year-old patient with a complicated crown fracture who underwent pulpotomy with mineral trioxide aggregate (MTA) and fracture fragment reattachment. An eight-year-old, systemically healthy girl presented to our clinic with a traumatic fracture of her maxillary right permanent central incisor. Clinical and radiographic evaluation revealed a complicated crown fracture and an open apex. A partial pulpotomy was performed using MTA to preserve the pulp. The fracture fragment, brought to our clinic 24 hours later in milk, was successfully fixed with composite resin following the pulpotomy. At 6 months of follow-up, no clinical or radiological symptoms developed, and the vitality of the involved tooth was preserved and its development continued. Partial pulpotomy using MTA is an effective and biocompatible option for vital pulp treatment in permanent teeth with open apexes. The reattachment technique increases patient satisfaction in terms of aesthetics and functionality and offers a minimally invasive approach. This case report demonstrates that complicated crown fractures can be successfully treated with conservative methods.
