ABSTRACT Background Metastatic tumors of the oral cavity are rare and generally associated with poor prognosis. Durable disease control after immune checkpoint inhibitor discontinuation is uncommon in clinical practice. Case Presentation Two patients with biopsy‐proven gingival metastases from lung adenocarcinoma received first‐line platinum, pemetrexed, and pembrolizumab. Pembrolizumab was discontinued after planned completion in one and because of immune‐related pneumonitis in the other. In both, gingival lesions completely regressed, and disease control persisted for more than 51.9 and 52.1 months without further systemic therapy. Conclusion These cases show that exceptionally durable disease control after pembrolizumab discontinuation can occur in patients with gingival metastases, both following planned treatment completion and after toxicity‐related discontinuation.
