The objective was to investigate the impact of HVs on the prognosis of patients with nm-RCC with TT after radical surgery. METHODS: Patients who underwent radical nephrectomy with removal of venous TT for a non-metastatic RCC were included in a retrospective study. Three groups were identified: clear cell carcinomas (ccCCR), papillary carcinomas (pCCR), and chromophobe carcinomas (chCCR). The primary outcome measures (disease-free survival and overall survival) were assessed using the Kaplan-Meier method and compared using the Log-Rank test. Univariate and multivariate Cox proportional hazards models were used to study the impact of histological variant on survival. RESULTS: A total of 873 patients were included. Histological subtypes were ccRCC, pRCC, and chRCC, accounting for 89%, 7%, and 4% of cases, respectively. At the time of data analysis, 612 (70%) patients were recurrence-free, and 228 had died (26%). Survival analysis showed a significant difference based on histological subtypes, with the worst survival outcomes for pRCC (p<0.05). In multivariable analysis, the presence of pRCC was independently associated with both DFS and OS (HR: 1.62; p=0.02 and HR: 1.76; p=0.03). Limitations include the retrospective nature of the study. CONCLUSIONS: In this multicentric series, histological variant appeared to impact the medium-term oncological prognosis of kidney cancer with tumor thrombus.
