Evaluation of Thyroid Functions in Patients with Chronic Kidney Disease and its Relation to the Disease Severity

Background: Chronic kidney disease (CKD) constitutes a significant public health concern. The prevalence of CKD in the general populations of different nations varies from ten percent to twelve percent. Aim: To evaluate the thyroid functions in cases with CKD and to clarify its relation to the disease severity. Patients and methods: This was a case-control research that performed on 90 Egyptian subjects above 18 years old. Results: There was highly significant reduction in FT3 level in group Ithan group II. Also, the level of FT4 showed a statistically highly significant decrease in group I than group II, while the level of TSH also showed a statistically highly significant increase in group I than group II. There was a significant negative correlation between FT3 levels and FBS, PPBS, and creatinine levels in patients of group I. FT4 levels were negatively correlated with creatinine and positively associated with eGFR levels. TSH levels were positively correlated with creatinine and HDL levels, but negatively associated with eGFR levels. Conclusion: Hypothyroidism is more prevalent in CKD patients, with free T3 and free T4 levels decreasing as CKD stage increases. Subclinical Hypothyroidism also increases in individuals with a decline in GFR. Monitoring thyroid functions is crucial for improving the quality of life in patients with advanced renal impairment.
