Retrospective study reports 92.1% survival rate of dental implants in reconstructed mandibles, suggesting strong osseointegration and favorable outcomes.
Purpose: This retrospective study aimed to evaluate the clinical predictability of dental implant treatment in the reconstructed mandibles with particulate cancellous bone and marrow (PCBM) and custom-made titanium mesh (Ti-mesh) tray for segmental mandibular defects. Methods: Thirteen patients, who underwent segmental mandibulectomy for tumor resection followed by mandibular reconstruction with PCBM and Ti-mesh tray, were enrolled in this study. According to the primary diseases, they were classified into group 1 (benign tumor) and group 2 (malignant tumor). Thirty-eight dental implants were placed in the reconstructed mandibles and investigated on the items as follows: cortical-bone thickness and CT value, tactile sense, implant stability quotient (ISQ) value, occurrence of peri-implantitis, and survival and success rates. The results were compared between the 2 groups. Results: Tactile sense was generally higher than the expected results based on the cortical-bone thickness and CT values. In fact, the median ISQ value at dental-implant placement was 78 and that at second-stage surgery was 84, suggesting successful osseointegration. During the follow-up period, 6 dental implants suffered from peri-implantitis and 3 dental implants (group 2) of them were removed caused to inappropriate plaque control. The survival and success rates were 92.1% and 86.8%, with significant differences between the 2 groups. No significant differences between the 2 groups were observed in other items. Conclusions: The clinical predictability of dental-implant treatment in our reconstructed mandibles with PCBM and Ti-mesh tray for segmental mandibular defects would be considerably high level.
