A BSTRACT Introduction: Diabetes ( Madhumeha ) is a group of diseases characterized by elevated blood glucose concentration because of diminished or exhausted insulin secretion from pancreatic cells and insulin resistance. Furthermore, diabetes is associated with factors that contribute to cardiovascular risk, including insulin resistance, dyslipidemia, hyperinsulinemia, hypertension, excessive oxidation, endothelial dysfunction, inflammation, and growth factors that affect platelet aggregation. The drugs available to control diabetes may cause serious side effects such as gastrointestinal disturbance, and renal and hepatic impairments for long-term usage. Hence, there is an urgent need for therapeutic measures that not only control drug dependency but also have less side effects. In Ayurvedic classics, 20 types of Prameha are described, and Madhumeha has been described as subtype of Vatika Prameha . Aims: The aim of the study is to compare the efficacy of Prameha Gaja Keshari Rasa and Virechana Karma in the management of Madhumeha . Materials and Methods: In this study, a total of 60 patients were taken randomly with computerized randomized method into two groups, with 30 patients in each group. Group A contains Prameha Gaja Keshari Rasa from Rasa Tantra Sara evam Siddha Yoga Sangraha who is given 250 mg BD before meal with lukewarm water for 30 days. In Group B Virechana Karma with Icchabhedi Rasa, Godugdha , and Erand Tail was given, followed by Snehapana with Panchatikta Ghrita . Results: In Group A, there are six instances of Complete Remission (76%–100% improvement), accounting for 21.4% of the total. Similarly, Group B has 19 instances of Complete Remission, representing a higher percentage of 65.5%. In terms of Marked Improvement (51%–75% improvement), Group A has 5 occurrences, making up 17.9%, while Group B has 9 instances, accounting for 31.0%. This study suggests that Virechana Karma become the promising therapy in prevention of type – 2 Diabetes Mellitus. Conclusion: Group B containing Virechana Karma with Icchabhedi Rasa, Godugdha and Erand Tail is better than Group A containing Prameha Gaja Keshari Rasa .
