Assessment reports significant PYLL due to traumatic brain injuries in Chelyabinsk, highlighting preventable mortality causes.
The indicator «Potential Years of Life Lost» (PYLL) is used in many countries of the world to assess social and demographic losses from premature mortality. It is used, in particular, by the World Bank, the Organization for Economic Cooperation and Development, and the World Health Organization. The PYLL indicator is also one of the modern tools for assessing public health losses, characterizing the number of deaths and the age of death, the latter is especially important for an adequate assessment of the significance of losses due to causes, perhaps not the most widespread, but inherent in young ages and classified as preventable causes of death. The assessment of the PYLL indicator plays an important role in shaping national strategies in the field of health and demographic development, and taking preventive measures to combat the causes of preventable mortality. Purpose: to calculate the rate of PYLL due to traumatic brain injuries (TBI) in the city of Chelyabinsk and the Chelyabinsk region in dynamics in 2011–2023 and to assess the economic damage caused to society by it. Materials and methods. The calculation of the indicator PYLL due to TBI was carried out based on the data of table C51 of statistical form № 5 «Distribution of the deceased by sex, age groups and causes of death» (table C51). Statistical processing of the obtained information was carried out using a personal computer and the statistical package of applied statistical programs SPSS Statistics (version 19) and MS Excel. Results. During the study period of 2011–2023, 8201 people aged 0 to 75 died as a result of TBI in the Chelyabinsk Region, and 2173 people aged 0 to 75 died in Chelyabinsk. Of all those who died in the Chelyabinsk Region, 80,27% were men (n=6583), and 19,73% were women (n=1618); in Chelyabinsk, 80,53% were men (n=1750), and 19,47% were women (n=423). When analyzing the PYLL due to TBI, it was noted that in the period from 2011 to 2023. Due to death from TBI, the population of the Chelyabinsk region annually lost from 25708,0 to 12966,5 person-years, the population of the city of Chelyabinsk annually lost from 6398,5 to 3102,0 person-years. In the city of Chelyabinsk, in comparison with the Chelyabinsk region, low values of the relative indicator of PYLL due to TBI are noted both among the entire population and among men and women. The level of the indicator among the total population in the city of Chelyabinsk in 2023 was 408,45 per 100000 population, in the Chelyabinsk region a similar indicator was 541,28 cases per 100000 population, which is 24,54% higher than in the city of Chelyabinsk. Conclusion. PYLL along with the mortality rate, serves as an indicator for assessing the health of the population. The PYLL calculation showed that there are significant reserves for reducing mortality from TBI, and clearly demonstrates the most vulnerable age groups, which is important when organizing medical care and planning targeted programs for injury prevention.
