A BSTRACT Left Ventricular pseudoaneurysm (LVP) is a rare mechanical complication following acute myocardial infarction (AMI) but is associated with high risk of sudden rupture. LVP is forms when the ruptured myocardium is sealed by the pericardium. It can present with congestive heart failure (CHF), arrhythmia, thromboembolic episodes and sudden death. Our patient presented with CHF class II after successful treatment of acute inferior wall myocardial infarction, complicated by cardiogenic shock and acute renal failure. Transthoracic echocardiography (TTE) showed large LVP along the inferoposterior and lateral walls. Although surgery was advised to him, patient’s family opted for medical management. We aim to highlight the importance of early diagnosis by clinical suspicion and echocardiography to prevent catastrophic outcomes.
