Due to the poor sputum excretion ability of patients undergoing lung cancer (LC) surgery, the accumulation of secretions leads to pulmonary infection (PI).Bronchoscopic alveolar lavage (BAL) can clear respiratory secretions, because local application of antibiotics can fight infection. This work explored the influences of BAL assisted mechanical ventilation (MV) on the clinical efficacy and inflammatory response of patients with postoperative PI.Ninety patients with postoperative PI were enrolled and rolled into MV group (n = 45) and a BAL-assisted by MV group (n = 45).The therapeutic effects, respiratory mechanics, lung function, blood gas indexes, and levels of inflammatory factors in peripheral blood were compared of patients in different groups.Based on the conditions in the MV group, the PI control time, MV duration, temperature recovery time (TRT), and PI score of patients in the BAL-assisted MV group were greatly reduced.Raw, PIP, and WOB decreased, while Cdyn increased.MMV, TLC, FEV1, and MMEF were enhanced greatly. In addition, PaO2, SpO2, pH, and PaO2/FiO2 increased, while PaCO2 decreased. The hs-CRP, PCT, TNF-α, HMGB1, IL-6, and MIP-1α in peripheral blood were decreased (all P< 0.05).The clinical effective rate (CER) of patients in the BAL-assisted MV group was higher to that in the MV group (93.33% vs 77.78%), showing P< 0.05.BAL-assisted MV in treating postoperative PI can achieve better results, effectively improve respiratory function and reduce systemic inflammatory response.
