In women of the reproductive age group, hemoperitoneum and a ruptured corpus luteum cyst are common, as supported by the study of Ho et al., where a total of 91 women in the age group of 15-42 years with a mean age of 26 years were diagnosed with ruptured corpus luteum cyst and hemoperitoneum, and 60.4% of ruptured cysts were found during the secretory phase. Unmonitored anticoagulant therapy can lead to an array of complications, one of which is intraperitoneal haemorrhage secondary to numerous medical and surgical conditions. Here by presenting a case of 32 year old women para two living two presented with abdominal pain and giddiness and was on anticoagulant for MVR with deranged prothrombin time and was diagnosed with warfarin induced hemoperitoneum with ruptured corpus luteal cyst. Ovarian function suppression should be discussed and prescribed to women in the reproductive age group who are not desirous of pregnancy and are on anticoagulant therapy along with uncontrolled bleeding. Strict monitoring of coagulation profile and ovarian suppression of these women can avoid such near-catastrophic events in these cases.
