The technically difficult superiorly based nasalis myocutaneous island pedicle flap has been described as a useful repair option for distal nose defects after Mohs surgery. Both the bipedicle and single pedicle modification have been described. With the use of a video demonstration the author demonstrates the method of preparing, raising and executing the single pedicle nasalis sling flap, with pre- and postoperative results of representative cases. The author also describes his 6-year experience with this flap with pearls on how to execute it for the best results.
