Background: Women with autoimmune inflammatory rheumatic diseases (AIIRDs) may be more vulnerable to human papillomavirus (HPV) infection and HPV-related cervical cancers 1,2. However, the HPV test and HPV vaccination rate was low in those patients 3. Objectives: We conducted a real-world survey to evaluate the HPV vaccination awareness and HPV vaccination status in Chinese AIIRDs patients. Methods: Female AIIRDs patients with age≥9 years were enrolled. From 1 April 2023 to 31 Aug 2023, eligible participants completed a predefined 47-question-based questionnaire by invitation on social media and visiting the outpatient department. We evaluated the demographics, rheumatic disease history, HPV infection and HPV vaccination awareness, HPV test, HPV vaccination status and HPV vaccination willingness of all the participants and compared the above status between SLE and RA patients. Results: 562 participants with age of 34.9 (29.7-42.6) years completed the relevant questionnaires. The disease distribution was demonstrated in Table 1. The awareness rate of HPV and HPV vaccination were 82.0% and 85.9%, respectively. Only 69.2% participants were aware of the increased cervical cancer risk associated with HPV. Only 52.3% patients underwent cervical cancer screening in the past 5 years. The willingness rate of HPV vaccine was 72.2%, however the vaccination rate was only 26.1%. 334 SLE patients and 98 RA patients were enrolled. Comparing to RA patients, the SLE patients were younger, had higher HPV vaccination awareness rate but a low vaccination rate (Table 2). Conclusion: Patients with AIIRDs have high awareness rate of HPV infection, HPV vaccination and high willingness of HPV vaccine but low cervical cancer screening rate and HPV vaccination rate.
