The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection causes a wide range of neurologic and hematologic manifestations. We report the case of a 69-year-old Indian male patient who presented with Guillain-Barre syndrome (GBS) and developed hemophagocytic lymphohistiocytosis (HLH) post-COVID-19 infection. He presented with acute ascending paraparesis with fever.Nerve conduction studies (NCS) revealed axonal degeneration affecting both motor and sensory nerves with albumino-cytologic dissociation in cerebrospinal fluid (CSF) analysis.Hemophagocytic lymphohistiocytosis (HLH) was considered due to the patient's fever, bicytopenia, and elevated levels of triglycerides and ferritin, along with the observed bone marrow. The patient received intravenous immunoglobulin (IVIG) therapy with a good clinical response.
