Purpose. To assess possibility and efficiency of femtosecond laser assisted traumatic cataract surgery in patient with severe zonular damage, lens subluxation and vitreous hernia. Material and methods. A patient after blunt trauma was diagnosed with traumatic cataract, severe zonular damage, lens subluxation with vitreous hernia, traumatic mydriasis, secondary glaucoma and vitreous opacification. Three-step surgery was performed: femtosecond laser assisted traumatic cataract surgery (capsulorhexis and phacofragmentation), phacoemulsification and IOL implantation, pars plana vitrectomy. Results. After surgical treatment optical media are clear, pupil is round, IOL is centered and stable, residual gas in the vitreal cavity, retina is adjacent. Vis OD 0.1 sul -1.5 ax 120 = 0.3, IOP OD 22 mmHg, B-scan: membranes are adherent, gas residuals in the vitreal cavity. Conclusion.Effectiveness of femtosecond laser assisted traumatic cataract surgery is shown in complicated case with severe zonular damage, lens subluxation and vitreous hernia.
