There is limited data on discharge trends post mitral transcatheter edge to edge repair (M-TEER). Due to the recent COVID-19 pandemic and the need to conserve resources, there has been greater efforts to shorten length of stay when possible. This study aims to evaluate the safety and feasibility of same-day discharge post M-TEER with the MitraClip platform. A retrospective review of patients undergoing M-TEER with MitraClip (Abott. Abbott Park, Illinois, USA) at our institution (Kettering Health) between 2016-2023 was performed. The patients were divided into two cohorts, same-day and next-day discharge. The endpoints of this study were time to discharge (TTD) and postoperative complications (readmission within 24 hours and 30 days, cardiac and non-cardiac death, acute myocardial infarction (AMI), acute congestive heart failure (CHF) exacerbation, cerebrovascular accident (CVA), acute kidney injury (AKI), new arrhythmias up to 30 days post procedure). A total of N=96 patients underwent successful M-TEER with MitraClip (NT, NTW, XT, XTW). 62.5% were male, with an average age of 79.68 years. TTD was compared between cohorts (45 patients next-day discharge and 43 patients same-day discharge). The same-day discharge cohort TTD was 0.95 days compared to next-day discharge with TTD of 1.89 days [p=0.0273]. Patients were safely discharged same-day with no statistically significant difference in 30 day readmission rates, 31.1% (next-day discharge) and 16.3% (same-day discharge) [p=0.1051]; although there was a trend towards less readmission in the same-day discharge group. There was no significant difference in 30 day deaths, 2.32% (same-day discharge) and 2.1% (next-day discharge) [p=0.9744]. There was no significant difference in 30 day adverse events (AMI, CHF exacerbation, CVA, AKI, arrhythmia). Same-day discharge for M-TEER using the MitraClip device appears to be safe, feasible, and can help mitigate healthcare strain and reduce resource requirements. Early discharge was not associated with increased complication rates, including 24 hour hospital readmissions or 30 day adverse outcomes.
