Endogenous hyperinsulinemic hypoglycemia cases are challenging to diagnose, and determining the etiology is imperative for treatment. We present a patient with hypoglycemia and pancreatic mass, and the workup required to reach the diagnosis of noninsulinoma pancreatogenous hypoglycemia syndrome (NIPHS).
