Cardiac implantable electronic device infective endocarditis (CIED IE), commonly caused by staphylococci, is morbid and costly. Diagnostic uncertainty complicates patient selection for device extraction. Pathogen specific cell-free DNA (cfDNA) is detectable in plasma even when conventional blood cultures are negative on antibiotics (abx). Longer duration of cfDNA detection is associated with complicated infections such as endocarditis. It thus may have a role in refining CIED IE diagnosis.
