METHODS: This retrospective, multicenter, analytical study included consecutive admissions of 62 patients diagnosed with prostate abscess between 2017 and 2023. Primary treatment consisted of antibiotics alone or in combination with Transrectal Ultrasound-Guided Aspiration (TRUS-GA) or Transurethral Resection of the Prostate with Abscess Deroofing (TURP-AD). Patients were assessed based on resolution with primary treatment and compared to those undergoing additional intervention. Additional intervention was defined as escalation to TRUS-GA, repeat TRUS-GA or TURP-AD. Further analysis categorized the sample into Prostate Abscess Complexity Score (PACS) distinguishing between Complex and Non-Complex. RESULTS: The study included 62 patients, with 47 (75.8%) responding adequately to the primary selected treatment and 15 requiring additional intervention (24.2%). Risk factors for additional intervention included: the presence of multifocal abscess (p=0.005), septa (p=0.023), and bilateral involvement (p=0.008). Moreover, an abscess diameter ≥3 cm (p=0.040) and a volume≥20 cc (p=<0.001) were observed as risk factors for additional intervention. The Prostate Abscess Complexity Score which included variables as Abscess focality, Septa, Laterality, Polarity, Abscess diameter, and Volume, demonstrated 73% sensitivity and 74% specificity in predicting additional intervention (p=0.001) when≥6 pts were used to classify prostate abscess as complex. Among prostate abscesses that were classified as complex, 80% required additional intervention, in contrast with those classified as non-complex, in which 34% required additional intervention (p=0.002). CONCLUSIONS: The identified risk factors, including multifocal abscess, septa, bilateral involvement, large abscess diameter, and increased volume, were identified as predictors of additional interventions. The proposed Prosate Abscess Complexity Score is valuable for clinical decision-making when≥6 points are encountered, emphasizing the need for tailored approaches based on individual characteristics.
