However, despite the high number of manuscripts and potential benefits described by some authors, the SP-RARP long-term functional and oncological outcomes are still lacking in the literature. In this scenario, we described our functional and oncological outcomes after four years of using this platform. METHODS: We reviewed the data of 122 patients with prostate cancer who underwent radical prostatectomy with the SP robot from June 2019 to July 2023, assessing perioperative results, pathology reports, long-term erection and continence recovery, and oncological outcomes. Medians and interquartile ranges (IQR) and frequencies and proportions were reported for continuous and categorical variables, respectively. RESULTS: Median follow-up is 35.8 months (23.8-36.5). The cohort had a median age of 62 years old (57-68), PSA of 5.8 (4.3-8.1), preoperative SHIM of 20 (14-25), AUA of 7 (3-11), and BMI of 27 (24.7-29.4). The median total operative time was 114 minutes (104-124), and the console time was 80 minutes (75-90). No intraoperative complications were reported. Median prostate weight was 48.5 grams (41-58). The overall positive surgical margins (PSM) rate was 13.9% (4.9% were pT2 and 9% were pT3). Overall, in patients who received full nerve-sparing and had a SHIM score >20, 59% achieved a trifecta (potent, continent, and PSA control). The median time to biochemical recurrence was 31 months (17.4-41.7) in 6.5% of the patients. CONCLUSIONS: This is the first study to report trifecta outcomes of radical prostatectomy with the da Vinci SP. The patients had optimal continence recovery and potency outcomes in candidates for full nerve-sparing are acceptable, but early potency recovery seems slower than in our multiport series. We had a small percentage of biochemical recurrence in the follow-up period, and most patients were pT3 in the final pathology. However, this robot has only five years of FDA clearance, and we still need a longer follow-up to achieve further oncological conclusions.
