Even in small tumors of <1 cm, marker expression is present in >80% of all cases. In collaboration with pathology, we are evaluating the test for detection of miR371a in patients undergoing inguinal testicular exposure for suspected testicular tumors in clinical practice. METHODS: Surgery was performed between 03/2022–08/2023 in 47 patients. Cubital vein blood is sent to molecular pathology within 30min for extraction of miR371a. The Maxwell RSC with the Maxwell® RSC MiRNA Plasma and Serum Kits is used here to extract and purify the miRNA. The extracted miRNA is transcribed into cDNA using cDNA Solution, Reverse Transcriptase and RNase Inhibitor. Subsequently, this cDNA is amplified. Using qPCR (Real Time-quantitative Polymerase Chain Reaction), the micro RNA 371a is quantified using Roche Lightcycler 480 II. Using the Lightcycler software, the median Cp (CP=crossing points) of the triplicates is calculated and converted into a RQ value (RQ=relative abundance). RESULTS: The evaluation of the miRNA assay showed a correct positive result with regard to histology for the total cohort of patients with inguinal exploration and either orchiectomy or organ sparing surgery in 70% of the patients (18x seminoma as well as 1x yolk sac tumor, 1x embryonal carcinoma, 1x somatic metastasis). False-negative findings were present in 30% of patients (8x seminoma, 1x embryonal cell carcinoma). A false positive result presented in one patient with teratoma, as well as in three patients with benign histology. A true negative result was identified in 81.2% of patients (9x benign histology, 2x Leydig cell tumor, 1x lymphoma, 1x Sarcoma). In comparison, the classic tumor markers (ß-HCG, AFP, LDH) were correctly positive in only 23.1% of the malignant histologies and false negative in 76.9%. The classical tumor markers showed no false-negative result. CONCLUSIONS: In the absence of suitable markers, miR371a shows promising results in the preoperative diagnosis for testicular tumors with uncertain histology and it is superior to classical tumor markers. However, a negative test does not rule out malignancy so that patients still need to undergo inguinal surgery.
